WHY IS INSURANCE NOT ACCEPTED AT PRAXIS PHYSICAL THERAPY?

• We treat using a more holistic approach with no barrier to treatment that certain insurance carriers will have for your care plan. In this case, YOU are in charge of your care and the alliance formed between yourself and your therapist will be a recipe for success. The price for each appointment is $160.00 for one hour. We believe that knowing exactly what you owe for the care you receive  takes the guesswork out and therefore, reduces the frustration and anxiety in today's medical climate.

CAN I SUBMIT MY TREATMENT RECEIPTS TO MY INSURANCE TO GET REIMBURSED?

• Since we are out-of-network, it is never a guarantee that your insurance will pay you back for services rendered. However, we can issue you a "SUPERBILL" which will include all the necessary information that a claims service agent needs. Call the customer service number listed on the back of your insurance card regarding submitting a Superbill so you can get reimbursed for the payment. Most insurance companies will have a form you can download online for this. Again, payment is never guaranteed. Be sure in inform the insurance company that they need to reimburse YOU, not the clinic.
